• Two identical passport photographs.
• State certified copy of your birth record or an old passport or an ORIGINAL Certificate of Naturalization (These will be returned with your new passport.)

Must complete lines 1-22 of passport application. Line 19 is optional unless expedited.
Remainder of application must be completed and signed in our office. For ages 16 and 17, the child and parent must appear and sign application.

If applicant is 16 or 17 years of age and has their own driver’s license or any form of a photo ID, they must present it, if not, the parent can use their ID.

For age18 and over, only the applicant has to sign the application and present their ID.

Cost must be paid by Check, Money Order or Cashier’s Check in the amount of $110.00 payable to U. S. Department of State and $25.00 payable to Iberia Parish Clerk’s Office for processing.

If Applicant is age 15 or younger, both parents or legal guardians must appear with their child and:  a) submit evidence of the child's U.S. citizenship, b) submit evidence that they are the parents/guardians, c) show valid personal identification, i.e. parent's drivers license. 

Note:  If second parent is not available, present evidence as to why non-applying parent is unable to appear for consent. (complete Form DS-3053 and have it notarized.)

Effective February 1, 2004: All minor children must appear in person to apply.

Cost must be paid by Check, Money Order or Cashier’s Check in the amount of $80.00 payable to U. S. Department of State and $25.00 payable to Iberia Parish Clerk’s Office for processing.

Must have been 16 years or older when issued previous passport to renew.

Must have two identical passport photographs and present old passport which is less than 15 years old from issue date.

Complete the front of the renewal form, date and sign the bottom of form.
Cost must be paid by Check, Money Order or Cashier’s Check in the amount of $110.00 payable to U. S. Department of State. Mail all of the above to: National Passport Center, P O Box 371971, Pittsburgh, PA 15250-7971.
